Customized Physician Database

1. Specify Which Fields Must Have a Value

Fax  Email 

You can choose to exclude physicians from your custom database if they are missing a fax number or email address. For example, if you select fax, then only physicians with fax numbers will be included in your database; if you select fax AND email, then only physicians with BOTH a fax and a email will be included; if you leave all boxes unchecked, then all physicians will be included, even if they are missing fax or email. Of course, selecting both fields will yield the smallest number of records, because only physicians with values for both fields will be included.

2. Select Primary Specialties (counts in parentheses)

Do not check any specialty if you want to include ALL specialties

3. Select Secondary Specialties (counts in parentheses)

179,460 of our doctors list a secondary specialty in addition to one of the primary specialties above. This gives you an opportunity to make a more specific selection. If you do not need a more specific specialization than one of the primary specialties above, you do not need to check any of the boxes below. In most cases, you will want to choose EITHER a primary specialty OR a secondary specialty but not both.

4. Select State(s) (counts in parentheses)

5. Select Counties

Enter the list of counties, separated by commas. Click on the Look Up link to select from list of counties with proper spellings. Make sure you select your state(s) first (above) in case the county name exists in more than one state.

6. Select Cities

Enter the list of cities, separated by commas. Click on the Look Up link to select from list of cities with proper spellings. Make sure you select your state(s) first (above) in case the city name exists in more than one state.

7. Select Zip/Postal Codes

Enter the list of Zip Codes, separated by commas. You can also use wild card (*) for partial zip codes or hypen (-) for a range of zip codes.

Examples:

• 24021, 24035, 24063 (3 distinct zip codes)
• 24021-24035 (range of zip codes)
• 24*, 25* (all zip codes beginning with '24' and '25')
• 24021, 25*, 26001-26050 (one zip code, all zip codes beginning with '25', and all zip codes between 26001 and 26050)

8. Select Area Codes

Enter the list of Area Codes, separated by commas.
